Sanjay Rajkot is the last living heir of the famous Rajkot royal family in Gujarat. The people admire him for his kindness and courage, calling him "Sikandar." He is married to Saisri, a talented painter whom everyone respectfully calls Rani Sahiba. Although Sanjay deeply loves her, his responsibilities often leave him with little time for their relationship.
During a flight, Sanjay witnesses Arjun, the arrogant son of powerful minister Rakesh Pradhan, harassing a woman named Monica. Monica is a former adult film actress who has rebuilt her life, married, and become a mother. Arjun threatens to expose her past to her son unless she agrees to sleep with him. Unable to tolerate the abuse, Sanjay fights Arjun and his bodyguards before forcing Arjun to publicly apologize to Monica.
Humiliated by the incident, Rakesh bribes Inspector Prakash to arrest Sanjay. When Prakash arrives at Sanjay's palace in Rajkot, he learns that Sanjay has already gone to the police station himself. By the time Prakash reaches the station, senior police officers order him to release Sanjay immediately, showing the respect Sanjay commands.
Meanwhile, Saisri feels lonely because Sanjay is always busy. He frequently forgets important occasions like their wedding anniversary and her birthday. One day, she discovers that she is pregnant and eagerly plans to tell him. At the same time, Sanjay learns that explosives stolen from one of his quarries may be used in a terrorist attack. He rushes to stop the danger without knowing that Saisri has secretly followed him.
A massive explosion occurs at the site, and Saisri is critically injured. She is taken to the hospital but dies before she can reveal her pregnancy to Sanjay. Before her death, she had registered as an organ donor. Her lungs, eyes, and heart are donated to three different patients in Mumbai.
Heartbroken, Sanjay travels to Mumbai to meet the people whose lives were saved by Saisri's final gift. He believes caring for them is the best way to honor her memory.
The first recipient is Kamar, a young orphan living in Dharavi. He suffers from severe lung disease because of the polluted air in his neighborhood. Sanjay discovers that businessman Virat Bakshi is deliberately dumping garbage on nearby land to force poor families to leave so he can build a shopping mall. Sanjay decides to stop Virat and improve the living conditions for the community.
The second recipient is Vaidehi Ranga, who received Saisri's eyes. Vaidehi runs a small vada business, but her conservative father-in-law refuses to let her work because he believes women should stay at home. Sanjay encourages the family to support her dream and helps her business succeed.
The final recipient is Nisha, who received Saisri's heart. Unfortunately, the transplanted heart begins to fail, leaving her in critical condition. Sanjay arranges for a heart specialist from Dubai to guide the local doctors through a live video call, helping them save her life. He later learns that Nisha's heart condition became worse after her former boyfriend, Kapil, cruelly informed her that he was marrying someone else. Furious at Kapil's heartless behavior, Sanjay confronts and beats him.
Just as Sanjay prepares to return home, Arjun spots him during one of Rakesh's political rallies. A dangerous chase follows through Mumbai. During the pursuit, Arjun accidentally drops Molotov cocktails onto his own car, causing an explosion that kills him.
Blaming Sanjay for his son's death, Rakesh swears revenge. He threatens to murder all three organ recipients to make Sanjay suffer. Refusing to abandon them, Sanjay stays in Mumbai to protect them.
Over time, Sanjay transforms the lives of all three families. He helps clean Kamar's neighborhood, allowing children to breathe cleaner air. Vaidehi gains her family's support and successfully expands her food business. Nisha recovers and enjoys a peaceful life with her loved ones.
Determined to destroy Sanjay's reputation, Rakesh frames him for a terrorist bombing. At first, the organ recipients begin to doubt him, believing the false accusations. However, Sanjay's friends uncover the truth and clear his name.
Soon afterward, Rakesh's men attack the hospital where Kamar is receiving treatment. Sanjay fights them and successfully protects the child and the hospital staff.
The following night, Sanjay storms Rakesh's mansion and defeats him in a final confrontation. After bringing the conflict to an end, Sanjay willingly surrenders to the police, believing he must respect the law despite everything that has happened.
His arrest sparks a massive public protest across Mumbai. Thousands of people gather to demand his release because of the countless lives he has changed through his courage and compassion. Faced with overwhelming public support, the authorities release him. By the end of the story, Sanjay fully earns the title "Sikandar," not because of his royal blood, but because of his bravery, selflessness, and dedication to protecting others.
